About Transitions

Overview
Transitions is a unique end-of-life care program that enables hospices to provide services to people with life limiting illnesses, even while they are still receiving curative care. By following our complete step-by-step implementation process, you will build a cost-effective service that will increase awareness of end-of-life care within the community. Ultimately, Transitions will allow you to help more people, earlier.

As a volunteer driven service, the overall cost of running Transitions is low and can be funded through community resources such as United Way, grants, memorials, and bequests. Your volunteers will provide compassion and specialized care through supportive relationships -- assisting people during an emotionally difficult time.

There are currently more than 80 programs throughout the country benefiting from the Transitions program. To learn more about what these programs have to say, see the Articles section.

Benefits
As a member of the Transitions community, your program will receive the following benefits:

* Comprehensive Training Transitions provides on-site training for all of your staff. After the intensive and customized training, you will be confident and ready to implement your own Transitions program. Staff-specific training ensures that everyone in your organization understands the program and their own role in its operation.
* Operations Manual Transitions provides you with a clear, easy-to-use Operations Manual that adheres to the strictest guidelines in the field of hospice care. All program providers continue to receive updates to the Operations Manual as necessary.

* Consultation and Follow-up You will continue to receive support free of charge. A complimentary follow-up phone consultation, scheduled six months after your program launch, analyzes your Transitions performance objectives. Our courteous staff is always willing to answer your questions and offer additional guidance any time you contact us.
* Program Directory You also receive a complimentary Transitions Directory containing listings of Transitions program providers and useful contact information for all Transitions coordinators and administrators nationwide.
* Networking Opportunities As an organization offering Transitions services you are invited to attend our Transitions Conference. Transitions coordinators, administrators and staff from across the country gather to discuss, plan and explore ideas regarding all aspects of program operation and sharing their "best practices." You will also be able to access our Member Forum, where you can share information with other Transitions programs.
* Confidence in our Product You can feel confident that Transitions is a program based on ethical practices and mission driven care. Transitions is the only program of its kind to be reviewed and approved by the Office of the Inspector General of the United States Department of Health and Human Services. Programs that provide free services to potential referrals, are frequently penalized under the Anti-Kickback Statute. The OIG concluded, however, that Transitions would not be subject to sanction under the Anti-Kickback statute. For further information, read the full OIG Advisory Opinion.
* Experience President and founder, Peter Briguglio, M.A., created Transitions in 1996. Peter's knowledge of the hospice industry is drawn from twenty-five years as a leader, administrator and advocate for hospice care. He has held the position of executive director for hospice organizations in several states. He was a founding board member of the Hospice of Saginaw (Michigan) and Hospice of the Straits (Michigan). He has served as a board member of the Maine Hospice Council, the Arizona Hospice Organization and the Hospice and Palliative Care Federation of Massachusetts. He is a nationally featured speaker.

On November 6th from 10-4 caregivers will be taken care of. We want to celebrate the CAREGIVER!! Day in and day out men and women are taking care of there loved ones and this is a day for them. Let us pamper you weather you want a massage or learn how to garden, this is a day for you. We will be all ready to pamper you at the Senior Center.
